Can your identity be stolen if someone knows your name, phone #, email, and address?

My friend and I were at the mall the other day, and some "scout" from Barbizon tried to get her interested in auditioning for the agency, so she filled out a form and gave her name, phone number, her email, and her address. She hasn't heard anything in a week and she's worried her identity can get stolen. As far as I know, as long as she didn't give her social security or credit card info she's ok. But she wanted me to be sure. Can her identity still be stolen since the people know this info?
